 of India, Ministry of Home Affairs, New Delhi.

 2. From the DPH&FW and Chief Registrar of Births & Deaths,  AP, Hyd., Lr.Rc.No.631/Dy.SO(CRS)/2014, dt.20.11.2014.

O R D E R:-
 In the circumstances reported in the reference 2nd read above, Government, after careful examination of the matter, hereby accorded permission to the Director of Public Health and Family Welfare and Chief Registrar of Birth & Deaths, AP, Hyderabad to incorporate the column of UID numbers of parents in case of Birth and still birth, UID number of deceased, parents of the deceased and spouse in Birth and Death reporting forms.
