ORDER:-

In the reference 1st read above the Commissioner of Health & Family Welfare, A.P, Hyderabad has requested to issue Administrative Sanction for implementation of NUHM in the State.

2. In the reference 2nd read above, Government have accorded administrative approval for implementation of National Urban Health Mission in Andhra Pradesh with the sharing pattern of expenditure between GOI and Govt. of A.P. in the ratio of 75% and 25% respectively.

3. In the reference 4th read above, the Commissioner of Health & Family Welfare, A.P, Hyderabad has furnished proposal for constitution of City Urban Health Society and EC for City Urban Health Society and also requested to

approve the MOU for entering between State Health and Family Welfare Societies and Respective City Urban Health Societies.

4. Government after careful examination of the matter here by constituted the City Urban Health Society and EC for City Urban Health Society and also approved the MOU with the following:-

2. Member of Parliament, representing City Member

3. Member of Legislative Council, representing City Members

4. Member of Legislative Assembly, representing City Members

5. Standing Committee Members Members

6. Two NGOs nominated by Municipal Commissioner Members

7. Heads of the Departments in Corporation Members

8. Superintendent/s of teaching hospital in City Member/s

9. Principal, Government Medical College/s Member/s

10. District Medical & Health Officer/s Member/s

11. District Coordinator/s of Hospitals Services Member/s

12. Commissioner Member/

EXECUTIVE COUNCIL OF CITY URBAN HEALTH SOCIETY:
